FOR PARTICIPANTS

Participant Application to join Scouting       For Participants - Age 14-20  
Print out, fill out, and mail to Dave ASAP.  Needed for the National Tour Permit below.

Medical Form   Required of all participants due to the nature of the trip.  A copy of a school Physical will also work for Part B. 
Please read part A- Health and Medical Record, B - Physical Examination, and Part C - Parental Informed Consent and Hold Harmless
Release Agreement. 
Required for BSA Supplemental Medical Insurance to be In Force for the Trip.
Must have on file prior to the Trip Departure.  Most Participants bring to the Spring Meeting. 

Parent Activity Consent Form  Use this form if using a copy of a school Physical and not filling out a Part C on the Medical Form.


FOR ADULTS AND STAFF MEMBERS

Adult Application      If you are 21 years of age or older.  Print out, fill out, and mail to Dave ASAP.  Needed for the National Tour Permit below.

BSA LEARNING CENTER  If you are 21 years of age or older you need to go to this Site, establish an Account and take
The following short classes:

  • Venturing Advisor Fast Start
  • Venturing Youth Protection Training
  • Safe Swim Defense
  • Safety Afloat
  • Trek Safely
  • Hazardous Weather
  • Climb On Safely


When finished, Print your Certificates and forward the Email or the link to them to Dave.  
 
BSA Transportation Page    All Adults and Staff Need To Read This Carefully.

National Tour Permit     American Odyssey must fill this out at least 60 days prior to departure to activate our insurance.
